This tracer patient methodology (tracer patient) is a quality tool for monitoring or evaluating a patient’s care in a health service, in order to detect the difficulties experienced by the patient in the different care settings and the functioning of the processes; there are two types of patient tracer, the individual patient tracer, in which the aim is to identify his or her particular experience of care and the patient tracer by topic or systems tracer (also called second generation patient tracer) in which cross-cutting situations are evaluated such as: prevention of intra-hospital infections, prevention of falls and pressure ulcers, medication management during their care.
This tool is widely used in the Joint Commission International’s health processes as a mechanism for verifying care and a source of improvement opportunities for health institutions. The collaborators that make up the team of internal quality auditors of the institution are:
